When you apply for a green card in the United States, you will be required to have a medical examination. The exam must be done by a doctor who is authorized by U.S. Citizenship and Immigration Services (USCIS).
Doctor Anna Morgan is a Civil Surgeon Licensed with USCIS services. She provides high quality updated Immigration Medical exams on a daily basis. She has proudly served the Chicago community for over 11 years and successfully completed hundreds of Immigration Physical Exams. In case of any complications with further proceedings we’ll follow your medical case closely, provide recommend treatment and document it in a proper way for USCIS.
All labs and immunizations will be done in one visit in our Clinic.
We speak Spanish, Polish, Russian, and Ukrainian.
Patients are advised to bring the following to their medical exam appointment:
Government-issued photo ID such as driver’s license or passport.
Vaccination or immunization record.
Any prior diagnostic tests.
Any mental health record.
A completed version of the most recent Immigration Physical Form I-693.
